Dotyczy formularza kontaktowego na stronie polybanking.app · Obowiązuje od: 01.06.2025

Zgodnie z art. 13 ust. 1 i 2 Rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z dnia 27 kwietnia 2016 r. (RODO) informujemy, że:

Administrator: administratorem Twoich danych osobowych jest Polava Michał Wajdziak, ul. Młyńska 22/1, 55-003 Nadolice Wielkie, NIP: PL9910339059.
Kontakt: w sprawach dotyczących danych osobowych możesz pisać na adres contact@polybanking.app.
Cel i podstawa prawna: dane z formularza kontaktowego przetwarzamy, aby odpowiedzieć na Twoje zapytanie i przygotować ofertę – na podstawie art. 6 ust. 1 lit. b RODO (działania na Twoje żądanie przed zawarciem umowy), a gdy działasz w imieniu firmy – art. 6 ust. 1 lit. f RODO (prawnie uzasadniony interes Administratora: kontakt z potencjalnym klientem).
Odbiorcy: dostawca poczty elektronicznej i infrastruktury serwerowej (serwery w Unii Europejskiej) – jako podmiot przetwarzający dane w imieniu Administratora.
Okres przechowywania: 6 miesięcy od wysłania formularza. Jeżeli zawrzemy umowę, dane będą przetwarzane zgodnie z Polityką prywatności.
Twoje prawa: masz prawo dostępu do swoich danych, ich sprostowania, usunięcia, ograniczenia przetwarzania, przenoszenia danych oraz wniesienia sprzeciwu wobec przetwarzania opartego na prawnie uzasadnionym interesie.
Skarga: masz prawo wnieść skargę do Prezesa Urzędu Ochrony Danych Osobowych (ul. Stawki 2, 00-193 Warszawa).
Dobrowolność: podanie danych jest dobrowolne, ale bez danych oznaczonych jako wymagane nie odpowiemy na zapytanie.
Przekazywanie poza EOG: nie przekazujemy danych z formularza poza Europejski Obszar Gospodarczy.
Zautomatyzowane decyzje: nie podejmujemy decyzji opartych wyłącznie na zautomatyzowanym przetwarzaniu, w tym profilowaniu.

Applies to the contact form on polybanking.app · Effective from: 1 June 2025

Pursuant to Art. 13(1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (GDPR), we inform you that:

Controller: the controller of your personal data is Polava Michał Wajdziak, ul. Młyńska 22/1, 55-003 Nadolice Wielkie, Poland, tax ID (NIP): PL9910339059.
Contact: in matters concerning personal data you can write to contact@polybanking.app.
Purpose and legal basis: we process the data from the contact form to answer your request and prepare an offer – under Art. 6(1)(b) GDPR (steps taken at your request before entering into a contract) and, where you act on behalf of a company, Art. 6(1)(f) GDPR (the Controller’s legitimate interest: contact with a prospective client).
Recipients: the provider of e-mail and server infrastructure (servers in the European Union) – as a processor acting on behalf of the Controller.
Retention: 6 months from sending the form. If we conclude an agreement, the data will be processed in accordance with the Privacy policy.
Your rights: you have the right to access your data, to have it rectified or erased, to restrict processing, to data portability and to object to processing based on legitimate interest.
Complaint: you have the right to lodge a complaint with the President of the Personal Data Protection Office (Prezes Urzędu Ochrony Danych Osobowych, ul. Stawki 2, 00-193 Warszawa, Poland).
Voluntary provision: providing data is voluntary, but without the data marked as required we cannot answer your request.
Transfers outside the EEA: we do not transfer data from the form outside the European Economic Area.
Automated decisions: we do not make decisions based solely on automated processing, including profiling.
